– Understanding the principles and process of Advance Care Planning (ACP)

– Understanding the implications of the Mental Capacity Act

– Advance Care Planning in clinical practice

– Developing effective communication skills

– Local resources to support individuals in the ACP planning process

– Demonstration of own role involvement and that of the multidisciplinary team
